Integrated Care Systems are not new organisations. They are intended to build and strengthen on existing partnerships and relationships across the health and care sector, to drive change, reduce inequalities and lead to better more joined-up care for the benefit of our population, as set out in the NHS Long Term Plan. Local Care Partnerships are the means at local level that give us the power to identify and focus our efforts on what matters to local people.
The Northern Local Care Partnership brings together health and care services with partner organisations to help people and communities in the Northern Devon locality live better lives. We cover a large area from Lynton on the North Coast to Holsworthy bordering on Cornwall. We serve a population of approximately 174,000 people.
We have a long and proud history of partnership working and integration of services and our Local Care Partnership builds on the strengths of our existing relationships, partnerships and networks.
