Our Team
We work with the small number of people who need extra health support or services above those normally provided by the NHS. The people who receive these services will have been assessed and have met the criteria for one of the funding streams we manage.
What is All Age Continuing Care (AACC)?
The aim of AACC is for individuals and families to have improved experience, transparency, and a smoother transition across all forms of NHS Continuing Care funding.
All Age Continuing Care (AACC) is an umbrella term covering different types of NHS Continuing Care funding, as listed below.
However please be aware that the policy frameworks for NHS Continuing Healthcare (for adults) and NHS Children and Young People’s Continuing Care (for those aged 17 and under) are currently distinct and separate.
