Population Health Management.

Population Health Management (PHM) is a way of working to help frontline teams understand current health and care needs and predict what local people will need in the future.  (NHS England)

Why do we need it?

So that we can tailor better care and support for individuals, design more joined-up services and make the NHS more sustainable.

How do we do that?

By using data to focus on a wide range of factors that contribute to poor health.  The data will be used to design new models of care that are proactive and make the best use of available resources.  The data that we use does not identify anyone and we never sell data to other organisations.

Where does the data come from?

In Devon, there is a database called the One Devon Dataset (ODD).  This dataset collects data from our partner organisations which may include GP surgeries, social care organisations, hospitals and voluntary organisations.  The information provided by this dataset can be used to create improvements in people’s health.
